請將申請表及下列有關資料交回通利琴行各門市部或郵寄：
九龍尖沙咀廣東道30號新港中心第一座11字樓「香港鋼琴/電子琴導師協會」收。
一經審核，申請者約於四星期內接獲通知。

1. 兩年年費共HK$100劃線支票 (抬頭「通利琴行有限公司」或 "Tom Lee Music Co., Ltd")
2. 音樂考試証書副本
3. 地址證明 (必須與申請表上的通訊地址相符)

Please send the completed form to Hong Kong Piano/Electone Teachers' Circle at
"11/F, Silvercord Tower 1, 30 Canton Road, Tsimshatsui, Kowloon" or
return it to any Tom Lee Music branch with the followings:

1. HK$100 membership fee for 2 years should be paid with crossed cheque made payable to
   "Tom Lee Music Co., Ltd"
2. Photocopy of certificate of music qualifications
3. Address proof (must be same address as in the application form)
